SAFETY FITNESS CERTIFICATE (SFC) APPLICATION
Application must be completed by the individual(s) named on the vehicle registration.
When registered to a corporation, it must be completed by a manager or officer of the corporation.
Applications submitted by third party consultants or auditors will not be accepted.
As of March 1, 2019, all commercial carriers applying for a new SFC must complete Alberta's pre-entry program requirements.

BEFORE you complete an application, the following should be completed:
- Safety and Compliance Course: The course must be completed by the appropriate individual.
If the vehicle(s) are registered to an individual, the appropriate individual must be the registered owner.
The appropriate individual for a corporation is a designated officer of the corporation.
- Knowledge Test: The Safety and Compliance Knowledge Test must be completed by the same individual that completed the course. Tests are available at any Alberta Registry Office.
- Safety & Maintenance Program: Programs must be prepared and implemented to provide policies and procedures for the safe operation of a commercial carrier.

To SUBMIT the application, you will need the following:
- MVID: The motor vehicle identification number (MVID) is located on your vehicle registration.
- Insurance: Information such as the insurance provider (not agent/broker), policy number, and amount of coverage.
- Credit Card: The application fee is $150 (Visa, Visa Debit, MasterCard, Debit MasterCard, American Express, and pre-paid credit cards are accepted).
